If you find a bird with the ring, and you were able to read its number, let us know about it. Please enclose the ring, if the bird was dead (you can flatten it; we can send it back on demand). After completing the data about the ringed bird you found, we will inform you giving its species, ringing place and date.

If you are not able to give exact answer to one or more of below's questions, please support approximate data or - if you cannot give any answer - put the question mark in the respective field.

What was the species of the bird?

The complete text imprinted on the ring (for colour rings - also location and colour).*

Date of record (finding, shooting, catching, observation).**

Place where bird was recorded: place, post, commune, province, voivodship.

Circumstances of the record (e.g. shot, found, caught, observed, ring number read through binoculars).

Status of bird at the moment of finding (healthy, injured, dead).

Reason of death or the way bird was caught.

If bird was dead, for how long time (e.g. fresh, decayed)?

If bird was caught, what happened to it later ? If it was released, where and when ?

All other information (e.g., age and sex of bird, additional circumstances of record).

Name of the finder, postal address.

E-mail of the finder.

Photo of the bird (important possibility of verification of the ring).
